This notice covers the smile assessment and callback form at invisalign-bondi.websited.org.
What the forms collect
Both forms collect your name, phone number and permission to respond. Email and your preferred callback time are optional. The assessment also collects your selected smile concerns, treatment timeframe, whether you are enquiring for yourself or your child, and any optional answers or notes.
You may choose to add up to three photos of your teeth. Photos are optional and require separate permission. If you are enquiring for a child, provide your own contact details as their parent or guardian and only share images you have authority to provide. Do not submit identity documents, payment details or detailed medical records.
Why the information is used
Better Smiles Bondi Junction and its website service provider, Websited, use your information to receive, administer and respond to your enquiry. The assessment helps prepare for a conversation. It does not diagnose a condition, confirm treatment suitability or book an appointment. These forms do not subscribe you to marketing.
How answers and photos are handled
Nothing is sent by the assessment until you choose “Send my assessment”. Unfinished answers and photo previews remain in the current page. We do not save them to our database or browser local storage. Keep the tab open to continue; your browser may retain form values through its own page restoration features.
Submitted information is stored in this website's dedicated database. Optional photos are checked and converted to JPEG, with embedded metadata removed, before storage. The original uploaded file is not retained. Photos are not published, used as advertising or passed to an image generation service.
Authorised staff and the website service provider can access submissions through a password-protected administration page. Database backups support recovery. If you request deletion, copies may remain in scheduled backups until those backups expire.
Website preferences and technical information
The page can remember your colour-theme preference in your browser. No advertising pixels or third-party analytics are connected. The form records submission time and, where present, campaign parameters and the referring website's origin to help identify the source of an enquiry. Technical logs and short-lived request controls help operate the site and limit spam.
External services
Online booking and map links open separate services with their own privacy arrangements. This website does not send your assessment to those services when you follow a link.
